Aims & Objectives (=What?) |
a) Aim: Reduction of burden of Human mortality and morbidity due to Emergency/Crisis of avian influenza/Bird-flu Objectives: 1. Provide clinical services, antiviral & other medicines by the public & private sectors health care facilities and providers during the epidemic. 2. Carry out surveillance and vaccination of suspected and confirmed cases 3. Implement strategies public health action for containment of the epidemic to ensure a prompt, coordinated and organized emergency response to Influenza pandemic by all stakeholders. |
Planning process/support required: (Risk/Vulnerability analysis) (=Where?) Pakistan is included in the list of the 45 Countries Currently in Crisis as Identified by the World Health Organization In the Health Sector especially in NWFP there exists no section or a cell which can meet the challenges of any types of emergency or crisis or epidemic(s)/pandemic(s) or disaster management especially for potential threat of Avian Influenza/Bird-flu H5N1. Avian Influenza/Bird-flu H5N1.the targeted type “A” influenza virus can infect several animals species including birds, pigs, seals and whales etc Influenza virus that infect birds are called avian influenza viruses. Waterfowls amongst the Wild birds are called as natural hosts for influenza “A’ virus, which causes contagious infection in domestic birds and leads to poultry epidemics which in turn weaken the economics and jeopardize food security, the greatest concern for human health is the risk that present condition could give rise to an influenza pandemic caused by the emergence of a new influenza virus H5N1 to which the general population have no or very little immunity and against which no existing vaccine will be effective. The emergency response is required to be generated for three phases; namelyPre Pandemic, Pandemic and Post Pandemic. I: Pandemic Emergency Response: i. A clear chain of command is essential for managing any emergency and maintaining control of daily operations until the crisis has passed. Therefore, the respective Health Authority will develop the command and control. a. It will also be vital to monitor the effectiveness of various plans, as they are implemented, and to adapt or refine them as and when needed to reflect the changing nature of the pandemic emergency. b. The Health Authority should enact community-based control measures for controlling the spread of influenza and minimizing its impact (i.e. closing community centres or schools, cancelling public gatherings). The main responsibilities of the Health Authorities are to: i. Maintain command and control structures in the Health Authorities and Institutions for emergency response ii. Liase with District Governments to coordinate, and gauge impact of, emergency response activities. iii. Adapt and refine Emergency Response Plans as needed. |
a. Post or person(s) responsible: (=Who?) Federal Emergency Coordination and Response Cell, headed by the Secretary Health having proper representation from Health and other sectors responsible for mitigating emergency. The office of the Deputy Director (Public Health), will be in charge of the operational processes whereas the Director Administration, DGHS will be overall in charge of the emergency response coordination and control Provincial /District Emergency Coordination and Response Cells, headed respectively by the Secretary of Health (having proper representation from Health and other sectors responsible for mitigating emergency) and the EDO(h) along with the disaster management Tea |
Standard Operating procedures (SOPs): Pandemic Emergency Response: A clear chain of command will be maintained, managing and maintaining control of daily operations until the crisis has passed. The Health Authority will enact community-based control measures for controlling the spread of influenza and minimizing its impact (i.e. closing community centres or schools, cancelling public gatherings). Specific Steps b. Operationalization of District Health Management Teams (DHMTs) with logistic support. c. Ensure bio-safety of the frontline Health workers. d. Establishing fever clinic/camps e. Upon pandemic notification from the health authorities, each health care facility will activate its Emergency Clinical Health Services Plan, which should ensure the continued provision of essential medical services and the appropriate care for influenza patients. f. During the pandemic, the MOH role in clinical services will be restricted to providing funds to the hospital and other health institutions, providing clinical recommendations and, if necessary, coordinating military assistance. g. The health authorities will develop guidelines to prioritise patients’ health care needs and Implement infection control measures. h. They will assess staffing levels and mobilize additional personnel as needed and securing alternate sites for providing medical care. i. The local media and the public will be informed about access to health services during the pandemic. j. |
Strategy/Triggering procedure / Authority (= When + Who + How?) 1. Declaration of pandemic will be the responsibility of the Director General Health Services on the Advice of the Director Health Services, DGHS NWFP. 2. EDO(H) will report on case load on a daily and need base. 3. 10% Case load will be used for declaration of emergency in a district. |
Early Warning + Communication (=Who + How?) Early warning will be issued in both directions (Up and Down) using all means of communication. Community early warning measures will be maintained by Lady health Workers |
Command , Control and coordination (=Who + How?) 1. Technical command and control will be maintained by the EDO(H) and the DHMT 2. The Deputy Director , Public Health will maintain Strategic Command and control of all response efforts |

Basic definitions of disaster management
Disaster
A serious disruption of the functioning of a society, causing widespread human, material, or environmental losses which exceeds the ability of the affected society to cope using only its own resources.
Emergency
A situation which poses an immediate risk to health, life, property or environment.
Hazard
Any climatic, geological, structural or environmental condition with potentially destructive effects (such as earthquakes, floods, landslides, droughts, etc.)
Vulnerability
Inadequate means or ability to protect oneself against the adverse impacts of natural events and, on the other hand, to recover quickly from their effects (physical, environmental, social and economic factors)
Disaster risk
The probability that a disaster may occur due to the presence of hazards and vulnerabilities.
The Disaster Management Cycle
Disaster Management cannot be limited to the reactive response to a disaster. The following phases are equally important:
Mitigation – to prevent a disaster from happening
Structural and non-structural measures to limit or impede the adverse impact of natural hazards, environmental degradation and technological hazards.
Preparedness – to minimize damage in case it is happening
Activities and measures taken in advance to ensure effective response to the impact of hazards, including early warnings and temporary evacuation of people and property from threatened locations.
Response – to assist the affected quickly and efficiently
All measures in the immediate aftermath of a disaster including the rescue and response efforts of the first minutes or often hours after the incident until professional emergency services come in.
Rehabilitation – to restore and improve the original situation
Rehabilitation provides the opportunity to “build back better”, to learn from the errors, hazards and vulnerabilities which may have contributed to the disaster and to mobilize all resources for restoring the original state.

A complex emergency is a humanitarian crisis in a country, region or society where there is total or considerable breakdown of authority resulting from internal or external conflict and which requires an international response that goes beyond the mandate or capacity of any single and/or ongoing UN country programme.
