What is New in IHR 2005?
• The routine measures for reducing the spread of diseases at airports and ports, existing guidelines on ship and airplane sanitation have been revised and updated.• IHR 2005 mandates establishment of “Real Time Event Management System’s by WHO and member states to address international health emergencies.
• IHR 2005 is no longer limited to notification of specific diseases. All health emergencies of international concern are to be notified to WHO in accordance with the decision instrument as given below at the bottom of this page
• WHO will coordinate the provision of technical assistance at the request of any member states; and work closely with it. WHO will also guide appropriate response actions.
• All member states will designate and maintain National IHR focal points to maintain a communication channel between WHO and countries.
Events Detected by National Surveillance System
A case of the following diseases is unusual or unexpected and may have serious public health impact, and thus shall be notified
The following diseases shall be notified to WHO under the international Health Regulations
o Small pox
o Poliomyelitis wild type Poliovirus
o Human Influenza Caused by a new subtype
o Severe Acute Respiratory Syndrome (SARS)
The following diseases shall or shall not be notified depending the criteria given for
Any event of potential International public health concern, including those of unknown causes or sources and those involving other events or diseases than those listed above or below in italics shall lead to the utilization of the algorithm
Any event involving the following diseases shall always lead to utilization of algorithm because they have demonstrated the ability to cause serious public health impact and to spread rapidly internationally
Is the public health impact of the event serious
If Yes
Then, is the event unusual or unexpected
If Again Yes
Then, Notify to WHO
Is the public health impact of the event is serious
If No
Is the event unusual or unexpected?
If Again No
Then, is there a significant risk of International Spread
If again the answer is No
Then do not notify the event at this stage. Reassess when more information becomes available
Is the public health impact of the event is serious
If the answer is no then proceed as below
Is the event unusual or unexpected?
If your answer is yes then
Is there a significant risk of international Spread
If Yes then Notify the event or Disease
but if there is no significant risk of international spread
but
there is significant risk of international travel or trade restrictions then Again notify the event otherwise don’t
